Your enrollment form asks new athletes your questions and has them accept your statements, such as a waiver. Edit it in Account settings › Enrollment form.
Turn it on or off
Tick Require before joining. Athletes then fill in the form and accept your statements before their request reaches you.
If it's off, athletes join without a form. They still give their name, email and date of birth, and you still approve each request.
Edit the questions
The form starts with a standard health check. You can:
- Change the Form title and Introduction.
- Edit a question's wording and pick its answer type: Short answer, Paragraph, Date, Time (hh:mm), Single choice, or Yes / No + details.
- For Single choice, type the options separated by commas.
- Tick Required so athletes can't send the form without answering.
- Move questions up or down, or delete them.
- Tap Add question for a new one.
Name, email and phone are always asked. Some questions show "Saved to athlete profile as…": those answers also fill in the athlete's profile or Medical ID.
Statements
Under Statements athletes must accept, each statement is a separate tick box, and all are required. Tap Add statement to add one.
Changing the wording creates a new version. Current athletes are asked to accept it again.
Save and preview
Changes show in a bar at the bottom. Tap Save changes (or Discard). Each save makes a new version. The top line shows the version and how many athletes have signed.
Tap Preview as athlete to see the form the way athletes will. Nothing is sent.
